Support coordination services
A support coordinator helps you understand your NDIS plan and explore and connect you with the right support and services based on your circumstances, needs and preferences to pursue your goals. DynaHealth is committed to working together with you to achieve your goals.
Our support coordinators will take time to get to know you, your goals and your needs and preferences to find the best solutions for you.
- Receive enquiry or referral (vIa phone or email)
- Make an appointment within 24 hours of receipt of contact (via phone or face-to-face or the mode of communication you prefer)
- Assess several providers including mainstream, community, informal options available
- Select preferred options or providers
- Negotiate services to be provided and fees
- Develop a service agreement and create service bookings with the preferred providers
